To be eligible, Entrants must:
- Be a current Valley Medical Weight Loss or Valley Medical Weight Control patient;
- Complete an entry form at any of our 3 locations;
- Weigh-in on one of the official weigh-in days – December 2-5, 2024; and
- Weigh-out between December 28-31, 2024.
All weigh-ins and weigh-outs must be documented by a physician or medical assistant employed by Valley Medical Weight Loss or Valley Medical Weight Control. Weights will be taken during business hours only. NOTE: Valley Medical Weight Loss will be closed December 24th, 25th and 26th. Total weight loss will be determined by the difference between weigh-in weight and weigh-out weight. The winner will be the entrant with the greatest weight loss between December 1, 2024 and December 31, 2024. In the event of a tie, the winner will be the entrant with the greatest weight loss as calculated by percentage loss (ending weight minus starting weight, divided by ending weight, multiplied by 100 ((EW-SW/EW) * 100 = weight loss percentage)).
By entering this contest, entrants accept and agree to be bound by these Official Rules. Any violation of these rules may, at Sponsor’s sole discretion, result in disqualification. All decisions of the Sponsor and/or judges regarding this promotion are final and binding in all respects. All federal, state and/or local income and other taxes, if any, are the winner’s sole responsibility. Not redeemable for cash or substitution. Good Luck!
*Free month will be based on Premier Program base price of $295, $395, or $525 value. Any upgrades for injections will require the winner to pay difference in value of price vs. total charged due to upgrade(s).
